The Manor ISD Athletic Department would like to thank you in taking part in our process of accepting athletic participation paper work online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.
- To access the online forms, click your curser over the “Electronic Participation Forms” tab and select “Athletic Participation Form Signature Page.”
- Begin by filling in your child’s name and student ID number as it has been registered with the district. This includes any hyphenated names. You MUST have your student’s ID number available, as it is required.
- Each blank must be filled. If no answer is known or needed, respond with N/A.
- You will have the opportunity to check a box to agree or acknowledge a document. You may click on it to view and have the option to print, although paper forms are not required.
- 5. To sign the document at the bottom, click inside the signature box and hold your mouse down, as this will allow you to create an “Electronic Signature”.
- Once you have filled out all of the information you will have the opportunity to print the entire document visible on the screen (this will not automatically include the UIL documents).
- You must include an email address at the very bottom of the document for the paperwork to be complete. You will receive a confirmation email once the Manor ISD staff has reviewed the document.
- All electronic forms must be filled out on a yearly basis.
- Physical forms are available to be downloaded and printed under the corresponding tab. These are required on a yearly basis and must be on file with the MISD Athletic Training staff. Please do not give to the front office, nurse’s office, or coaching staff. Incoming 9th grade students are responsible for securing a copy of their physical if it was turned in to the middle school. It must be turned in to the high school.
A STUDENT WILL NOT BE CONSIDERED CLEARED FOR ANY ACTIVITIES UNTIL ALL OF THE ITEMS HAVE BEEN APPROVED.
